This is a participation agreement that Apprenticeship Maryland requires students (incoming youth apprentices), employers, parents/guardians, and coordinators to sign. The first page of the agreement is a form that provides space to list contact information for all parties, student schedule, and employer worksite information. The second page outlines the responsibilities of students, employers, and the coordinator.
